Who is this for?
Catechized adult Catholics, age 19 and older, who meet the following conditions:

- Baptized and have received First Communion.
- Active and participating members of their parishes – highly motivated to celebrate the sacraments.
- Aware of the major doctrines and practices of the Catholic faith -committed to living as disciples of Christ.

DRE’s are Always welcomed to join in the conversation!
What documentation is required?
The following documents are required at the beginning of the preparation period and prior to the celebration of confirmation. Paperwork should be submitted to the parish where the candidate is currently registered and electronic copies sent to Joann Reier.

- Current baptismal certificate with updated notations OR copies of all sacramental certificates.
- Affidavit of eligibility signed by the sponsor with signature/seal from the parish where the sponsor is registered.
- Letter of permission from Pastor (REQUIRED) for candidate to be prepared in this program.
- If there is no First Communion Certificate – photos of the celebration of the sacrament as proof or Affidavit of a Witness.

Information For Parishes:
- This preparation journey is not intended to take the place of the parish’s program, but simply an aid for those parishes that do not have anyone available to offer preparation for those catechized adults desiring to complete their Sacraments of Initiation.
- Confirmations are to be recorded in the Sacramental books at the parish of the Confirmation Candidates (Confirmandi), not at the parish where the sacrament was given. The Candidates (Confirmandi) parish is also to send the notification to the parish of Baptism.

This celebration of Confirmation is for individuals 19 years and older who have been baptized and have previously received First Communion. Remember to
choose sponsors BEFORE you submit the candidate forms; only one Sponsor per candidate per the Archdiocesan guidelines.
The Office of Worship will be sending out the candidate forms and other information mid September. A reminder all information will be sent to Pastors and Parish Offices through their Archdiocesan email.
